To request an accommodation that allows you to be exempted from a vaccination as required because of a clinical site placement, please complete this form, upload it to the BC portal, and provide it to your Program Director. In general, exemption requests must be based on valid medical or religious grounds. The College will review your request and notify you, in writing, whether it is approved. The College may ask you for additional information about your request, and you may submit additional information. You may also be required to submit a vaccination exemption request to your clinical site.
