Legal / Home / Permanent Address Information

For a Home Address update, you will be required to attach one of the following:

Driver's license updated to include new mailing address, OR
Driver's license (not updated) AND an official document with your name and NEW address on it, such as a utility bill, credit card statement, or mortgage/lease agreement.
